Consent and Confidentiality and Electronic Communication Consent

I understand that all information gathered by FRAFCA programs is considered confidential and will only be shared outside FRAFCA if I have given my written permission. I also understand that in some situations, information about me may be disclosed because it is our policy or is legally required. Information could be released without written permission in the following circumstances: Responding to a threat of harm to self or others, responding to a medical emergency, reporting of suspected child abuse, complying with a subpoena. In order to communicate via email or text we must inform you: We cannot guarantee the security and confidentiality of messages sent electronically, we cannot guarantee that your email will be received, read, or responded to within any particular period of time, email, text, and instant messages can be intercepted, misdirected, and falsified, it is not possible to verify the true identity of the sender, or ensure that only the intended recipient will view the message, I understand that I am solely responsible for the security of E-mails and text messages I send and receive. I have read and understand the above information, and do herby hold harmless FRAFCA for any loss, cost or damages allegedly sustained by me due to the release of information under the circumstances stated above.
